Managing Side Effects on TRT

Most side effects on TRT are mild, expected, and manageable — and the ones that aren't have early warning signs we actively watch for. This guide covers what's normal on testosterone treatment, what's not, and the one rule that keeps you safe.

The common ones

  • Acne or oily skin — common early on as your levels settle into their new range. It usually eases on its own; mention it at your review if it doesn't.
  • Injection-site soreness — a day of mild soreness after injecting is normal. Spreading redness, heat, or a hard painful lump is not — message us before your next dose.
  • Water retention — a little puffiness in the first weeks isn't unusual and typically settles as your body adjusts.
  • Mood or sleep changes — most men feel more energetic and clearer early on. If you feel irritable, flat, or your sleep deteriorates, tell us — it's often a sign your dose or balance needs a small adjustment.

The quiet ones: what your bloods catch

The two side effects that matter most on TRT are ones you can't feel. Your red blood cell count can creep up, and your oestrogen balance can drift — both happen quietly, and both are easy to manage when they're caught early. The golden rule: don't adjust your own protocol

If something feels off, the answer is never to halve a dose, double a dose, or stop cold on your own. Your dose is set against your bloods and your history — changing it blind can make the side effect worse or create a new one. Message us first. Adjusting your plan is a normal part of treatment, and it's quick when we do it together.

Get urgent help first if

Severe allergic reaction, trouble breathing, chest pain, or anything that feels seriously wrong — call 000 or get to an emergency department first, then let us know.

Message the care team if

  • A side effect is bad, getting worse, or hasn't settled after a couple of weeks.
  • An injection site is red, hot, swollen, or has a painful lump.
  • You're tempted to change or skip doses because of how something feels.
